Today, progress in ophthalmology is very closely linked to new technical developments and without new and innovate contributions from equipment, instrument, lens and pharmaceutical manufacturers and also the many conservative and operative techniques employed, ophthalmology would not be in the excellent condition it is in present. Therefore the presence of the industry here radically enriches our joint SOE & DOG meeting and more than warrants a visit by delegates.

For limb reduction defects, our findings are generally consistent with those of earlier reports which suggested reductions in risk for periconceptional or early first trimester use 5, 7, 10 ; , with the greatest effect being evident for periconceptional use 5, 10 ; . Two of the earlier studies divided limb reduction defects by type of limb deficiency; one found that the effect was confined to limb reduction defects other than longitudinal deficiencies 10 ; , while the other found that the effect was confined to limb reduction defects other than transverse deficiencies 7 ; . Unfortunately, there were too few limb reduction defects in the present study to examine these subtypes. A case-control study of urinary tract defects and nonmalformed controls 9 ; found statistically significant reductions in risk associated with supplementation before, during, and after the first trimester. In addition, the previously described randomized trial 5 ; observed one case of obstructive urinary tract defects and no cases of renal agenesis in the supplemented group, as compared with three cases of the former and two cases of the latter in the nonsupplemented group. The present study confirmed these findings in that risks for urinary tract defects overall and for the subgroup of obstructive defects were generally below 1.0 for all timing categories and were significantly reduced for pre-LMl and LM2 supplementation. There were too few cases of renal agenesis in the present study to examine them separately. In the same randomized trial 5 ; , fewer cases of congenital hydrocephalus and pyloric stenosis were observed in the supplemented group than in the nonsupplemented group. We estimated some risks of congenital hydrocephaly to be below the null value, but we found no statistically significant reductions in risk or informative patterns of risk across gestational timing categories. For pyloric stenosis, we found that periconceptional use or first use in LM2 did not reduce risk, but later first trimester use did. The possible benefit of relatively late exposure is consistent with the randomized trial in that women in the supplemented group were started on multivitamins before conception and use continued at least through the first trimester. Furthermore, the findings are of interest in that the gestational timing of pyloric stenosis is not known; but the defect is thought to result from a functional disorder of the pyloric sphincter, which suggests that it may arise later in gestation than most other congenital defects 19 ; . For the overall data, we considered possible sources of error. First, we attempted to reduce the potential for information bias with rigorous data collection, including use of a standardized questionnaire administered within 6 months after the date of delivery. All of the aforementioned investigations, which identified either tolerance or reverse tolerance i.e., sensitization ; , were conducted in reproductively mature subjects. Although adolescents and young adults frequently use Ecstasy Pope et al., 2001, Lieb et al., 2002; Degenhardt et al., 2004; Jacobsen et al., 2004 ; , only recently have results with controlled animal studies begun to characterize the physiological, neurochemical, and behavioral consequences of MDMA exposure during this important developmental period Spear, 2000 ; . Initial work by Broening et al. 1995 ; found that both the thermal and neurotoxic effects of MDMA in rats increase with age from infancy through adolescence to adulthood. Although subsequent studies have confirmed that younger animals are less sensitive to MDMA neurotoxicity than adults Kelly et al., 2002; Meyer et al., 2004 ; , adolescent exposure to this compound has been found to cause measurable decreases in 5-HT levels as well as serotonin transporter SERT ; binding Fone et al., 2002; Morley-Fletcher et al., 2002; Bull et al., 2003, 2004; Piper and Meyer, 2004; Piper et al., 2005 ; . Treatment with MDMA during adolescence has also been shown to produce a variety of functional consequences. Short-term treatment regimens resulted in a later decrease in social interaction but an increase in cocaine reward measured using place conditioning Horan et al., 2000; Bull et al., 2004 ; . Finally, our group has shown that intermittent MDMA administration throughout adolescence in rats can elicit tolerance to the temperature dysregulation and 5-HT syndrome responses to the drug Piper et al., 2005 ; and can also affect performance on later tests of working memory and anxiety Piper and Meyer, 2004 ; . The present study was designed to extend our previous work involving an intermittent adolescent dosing regimen in rats Piper and Meyer, 2004 ; . In the first experiment, we examined the influence of this treatment regimen on the behavioral, physiological, and neurochemical effects of a subsequent MDMA "binge" administered in young adulthood. Of particular interest was the question of whether the adolescent preexposure would increase or decrease the animals' vulnerability to the neurotoxic effects of the binge treatment. The second experiment assessed whether adolescent MDMA administration would alter 5-HT1A-mediated functions, based on the involvement of this receptor subtype in temperature regulation and the serotonin syndrome Aguirre et al., 1998; Granoff and Ashby, 2001; Hedlund et al., 2004.

The main finding of this study is that early treatment with enalapril in patients with evidence of myocardial cell injury TnI increase ; after HDC seems to prevent the development of cardiotoxicity and the occurrence of associated adverse clinical events. In previous studies, we demonstrated that the increase in TnI soon after HDC is a strong predictor of cardiotoxicity and poor cardiological outcome, with the highest risk observed in patients showing a persistent 1 month ; TnI increase.5, 6 Identifying such a high-risk patient subset highlights the opportunity for targeted preventive measures. Much of the confusion surrounding thiamin therapy may be attributed to the fact that high sulfate intake may induce pem through either one of, or a combination of, two distinct mechanisms.
First, a simple interpretation of the given data is complicated by a large variability in methodology and evaluation criteria. Some authors use a battery of tests including functional assays such as reduction of nitroblue tetrazolium or a-naphthyl acetate esterase ; , phagocytic activity, appearence of specific surface receptors, morphological changes and loss of proliferative capacity whereas other authors use only one or two assays. Also, some authors may consider differentiation to have taken place if 20%-30% of the cells are positive with respect to a single differentiation marker or if morphological changes have taken place, whereas others interpret the same data as negative results. Second, a given treatment may be scored as a false negative because the pluripotent parental cell line differentiated along a different pathway than the one examined. For example, human K 562 cells are pluripotent and can be induced to differentiate into either and thiothixene.
Additional thiamin supplementation beyond the recommended daily allowance is generally not considered necessary in women taking oral contraceptives!

Thiamin dosing
Carbon 13 NMR spectra have been obtained for aqueous solutions of oL-2- tu-hydroxyethyl ; thiamin, DL-2- CV-hydroxybenzyl ; thiamin, DL-2- a-hydroxybenzyl ; oxythiamin, and related N-3 methyl and N-3 benzyl analogs. The unusually large downfield shift of the 13C resonance of C-2 of hydroxyethylthiamin suggests that this carbon bears a partial positive charge. This result stands in contrast to results of x-ray crystallographic studies of hydroxyethylthiamin, which place a partial negative charge on C-2 Pletcher, J., and Sax, M. 1974 ; J. Am. Chem. Sot. 96, 155-165 ; . A partial positive charge on C-2 helps to explain the facility of carbanion formation at the cy carbon both enzymatically and in model systems. The rates of proton-deuteron exchange of C- U ; -H with solvent deuterium, and of release of aldehyde to regenerate thiamin have been measured for hydroxyethylthiamin and analogs. The differences in kinetic acidity of C-n ; -H and of rates of aldehyde release are rationalized in terms of differing electron-withdrawing abilities of the substituents attached to N-3, and appear not to be related to intramolecular basic catalysis of these processes by the C-4' amino group
Young children can be caused by PCM and many other illnesses. In this study, because thiamin deficiency was not suspected and was a surprise fmding in both healthy and malnourished children, careful sensory testing was not performed although no loss of deep tendon reflexes which were examined ; was found. It is fully agreed that careful neurological examination should be done in any future study of thiamin status 16 ; . The etiology for the biochemical thiamin deficiency uncovered in this study has not been elucidated. Detailed quantitative dietary data was not obtained. A review of diet surveys in Ghana from 1968 compiled in reports by Whitby 1 ; showed that dietary thiamin intake of 50 to 80% of requirements occurred in some of the coastal fishing and forest villages and in some of the forest farm areas. Dietary thiamin intakes, mainly in adults in the Accra region where this study was also conducted, were found then to be adequate. Thiamin intake, however, may well be marginal in young, growing children. In this study the most marked elevations of percentage of TPP effect were found in the apparently normal groups. Actively growing children have higher thiamin requirements than malnourished children with little or no growth 13 ; . Since thiamin requirements are proportional to carbohydrate intake 13 ; and the total caloric intake of the normal children no doubt exceeded that of the malnourished and tiagabine.

The major objectives of the survey could not have been accomplished without the valuable assistance of Claudio Chehebar of the Argentinian Andino-Patagonico and Fueguino National Parks Service and the chief park officials at each of the park units visited. The latter not only provided information and logistical support such as the use of boats but also were pleasant and exceedingly cooperative field companions. We are especially indebted to Laura Fenoglio, Mario Cardenas, Horacio Giacchino and Marcos O'Campo of the National Parks Service and Mario Rajchenberg of CIEFAP Forestry Service in Esquel. The overall survey was not only a great success in terms of the scientific data collected but also represented an incredible journey that left each of the participants with memories of such things as the seemingly tons of dust generated on the unpaved roads, the toppling winds of El Calafate and Ushuaia, the thundering of cracking glaciers at Perito Moreno and Tronador, the silence of the mountains, the summer snow in Challhuaco and the peaceful beauty of reflected peaks in the glassy mountain lake of Puerto Blest. All these were experienced under the wings and in the graceful shadows of the condor.
