FINANCIAL REVIEW Trading profit and earnings per share Business performance trading profit grew 8% to 1, 891 million on turnover growth of 3%. The trading margin improved 1.6 percentage points to 35.2% compared with 33.6% in Q2 2002. This improvement came principally from cost of sales, which declined by 4%, and selling, general and administration S, G&A ; costs, which declined 1%. R&D expenditure increased 12%. Cost of sales reduced by 1 percentage point on sales, this benefit principally coming from continued merger, manufacturing restructuring and other cost savings. S, G&A expenditure reduced by 1.6 percentage points on sales as a result of cost savings arising from merger implementation and the operational excellence cost saving programme. Net other operating income was 87 million in the quarter compared with 16 million income in the second quarter of 2002. This included the disposal of a number of minor non-core products. Business performance EPS of 23.9 pence in the quarter increased 15% in CER terms and 9% in sterling terms. The adverse currency impact on EPS of 6% in the quarter reflected the continued weakness of the US dollar relative to last year partly offset by a stronger Euro. Statutory results, which include merger and manufacturing restructuring costs, delivered a trading profit of 1, 801 million on turnover of 5, 375 million. Taken together with other expenses, taxation and product divestments this resulted in EPS of 22.8 pence compared with 19.7 pence in the second quarter of 2002. Merger and manufacturing restructuring costs were lower in the quarter than in Q2 2002, reflecting movement towards the completion of these programmes. The sterling based growth in EPS of 16% was higher than the CER based growth in business performance EPS as a result of lower merger and restructuring costs partly offset by the overall negative impact of currencies. Merger and restructuring Costs of 83 million were incurred in the quarter in respect of merger and manufacturing restructuring compared with 185 million in the same quarter last year. GSK remains on track to deliver forecast total annual merger and manufacturing restructuring savings of 1.8 billion by the end of the year. These costs and benefits exclude the effect of the Block Drug acquisition. Currencies The second quarter 2003 results are based on average exchange rates of 1 .62 and 1 Euro 1.43 which compare with average rates for the second quarter of 2002 of 1 .47 and 1 Euro 1.58. Exchange rates at 16th July were 1 .59 and 1 Euro 1.42. If exchange rates were to hold at these current levels for the remainder of the year the impact will be broadly neutral in the second half of 2003. Dividend The Board has declared a second interim dividend of 9 pence per share. This compares with a dividend of 9 pence per share for Q2 2002. The equivalent dividend receivable by ADR holders is 28.637 cents per ADS based on an exchange rate of 1 .59095. The dividend will have an ex-dividend date of 30th July 2003 and will be paid on 2nd October 2003 to shareholders and to ADR holders of record on 1st August 2003. Earnings guidance for 2003 GSK's earnings guidance for 2003 has been improved to high single digit or better percentage growth in business performance earnings per share at constant exchange rates, regardless of possible generic competition to Paxil in the USA see "Legal proceedings" on page 19 ; . Share buy-back programme In October 2002 GSK commenced a new 4 billion share buy-back programme. Of this new programme, 219 million was spent in 2002, 262 million in the first quarter and 275 million in the second quarter of 2003. The exact amount and timing of future purchases will be determined by the company and is dependent on market conditions and other factors.

A significant minority of patients remain uncontrolled on three agents A + C and their management can prove difficult. Simple interventions like fixed dose compound preparations of drugs may improve compliance. A search should be made for substances prescribed or over the counter non-steroidal antiinflammatories, "herbal" remedies ; that may be having a deleterious effect on blood pressure control. Lifestyle advice should be reiterated as excess sodium intake may be a significant factor in limiting the antihypertensive action of drugs. In addition however, consideration should be given as to whether the patient may have a secondary cause for their hypertension Table II ; . In particular, recent recognition of the increased prevalence of aldosterone associated hypertension34 provides a rationale for screening for relative aldosterone excess using the aldosterone to renin ratio ARR ; . Interpretation of ARR can be problematic particularly if there is concomitant use of antihypertensives which cause suppression or elevation of the plasma renin activity PRA ; Table III ; . Once an elevated ARR has been established, the question as to whether excess and autonomous aldosterone arises from an adrenal adenoma is raised; as a result, further investigation of possible primary aldosteronism should be within a secondary care centre. The potential benefit of these investigations lies in the fact that resistant hypertension due to idiopathic hyperaldosteronism can be effectively managed with addition of the aldosterone antagonist spironolactone.

Assuming a cost of 00 US per stent and 50% usage, the hospital will lose .8 million with this transition. If DES usage approaches 70% or 80%, costs will escalate proportionately. Although patients and insurance carriers benefit by having fewer repeat interventions and fewer bypass operations, hospitals face a "triple whammy." Fewer repeat procedures and fewer bypass operations will cause less revenue to be generated, and the higher DES cost will cause enormous increases in supply costs. Until a more comfortable equilibrium is established between costs and reimbursement, cogent arguments have been forwarded suggesting more restricted use of DES, even to the point of strict rationing. Clearly, these very real economic concerns are the immediate "dark side" of DES euphoria. In fact, the slow acceptance of these stents in Europe is overwhelmingly related to the prohibitively high cost.

The latest treatment guidelines published by the STD AIDS Programme dates from June 2006 see table 4 ; . These guidelines present some innovative and bold features concerning the preferential therapeutic schemes to be applied. The Brazilian AIDS Programme has opted for adopting newer, therefore patented, antiretroviral drugs from the very beginning of the treatment. Such is the case of the antiretroviral tenofovir TDF ; which is indicated as a preferential drug in the early stages of treatment32 given its better therapeutic efficacy, its simplicity of administration with a single daily dosage, and its excellent safety profile. It is important to note that TDF has been added as component in the first-line treatments in the latest WHO guidelines and that this drug has become the most prescribed treatment in industrialised countries. Similarly, a mounting number of patients have been adopting lopinavir boosted with ritonavir LPV r ; Abbott ; and atazanavir Bristol Meyer Squibb ; as an alternative to older drugs of the same therapeutic category named Protease Inhibitors ; Previously, both TDF 2003 ; and LPV r 2002 ; have been comprised within the Brazilian treatment guidelines for patients under second line treatments. Nevertheless, the number of patients using this drug has greatly increased, given the recent introduction of these drugs in the first line regimens, as well. It is important to note that LPV r is prescribed for many years as component in preferential treatments in industrialised countries and that this drug has also been recently included in WHO treatment guidelines, but as key element in second-line treatment. Another preferential treatment that has been employed for long consists of the use of Efavirenz Merck ; , as one of the components of the preferred triple therapy. [inser table 4] The main issue posed by these protocols relies on the ambiguous situation consisting, on one hand, of the introduction of these new drugs providing a better clinical response to the HIV AIDS, and escalating costs on treatment, on the other. Presently, it is observed an acute imbalance in the disbursement of the National budget for ARV procurement. Approximately, 80% of the Brazilian Ministry of Health's budget is spent in the procurement of imported patented drugs. Amongst them, almost 65% of this budget corresponds to the acquisition of tenofovir 12, 2% ; , lopinavir r 34, 5% ; and efavirenz 17, 8% ; . According to the estimations of the Ministry of Health, the cost to purchase these 3 drugs might increase 2fold, until 2011 Cohen, 2006 ; . By comparison the remaining 20% of the budget are addressed to the purchase of 10 antiretroviral drugs that are locally manufactured. These numbers show the economies derived from local manufacturing of generic ARVs and robitussin.

If you are taking both amprenavir and the hiv drug ritonavir norvir ; , you must be careful to avoid the heart medications flecainide tambocor ; and propafenone rythmol. Edema loss ; 1-6 only one study showed that the fluid lost had the composition of extracellular fluid.18 In these reports, the fluid lost was described in terms of extracellular and intracellular fluid losses, and shifts of ions from one compartment to the other. By this interpretation, the proportion lost from each compartment varied from patient to patient, and there was water loss that could not be ascribed to either compartment. In the present study, by using the loss of sodium per kilogram of weight loss, two patterns occurred and within each pattern the findings were quite consistent. Table 1 shows the two groups of patients, separated on the basis of sodium loss per kilogram of weight lost. The 10 patients in the first group lost approximately 100 mEq. Na Kg., and the five patients in the second group lost less than 50 mEq. Na Kg. In group I the edema was secondary to venous stasis or hypertensive arteriosclerotic, or beriberi heart disease. The results of the three patients in this group whose edema was due to venous stasis were essentially the same as the results from the patients whose edema was due to and rocephin.
Chronic infusion 30 days ; of ANG II in apoE-KO mice resulted in an increase in systolic blood pressure by 36 mmHg 160 12 vs. 124 4 mmHg, n 47 group, P 0.05 ; and carotid plaque area by sixfold 34.6 3.6 vs. 5.0 1.9%, n 410 group, P 0.001; Fig. 2 ; compared with vehicle-treated apoE-KO mice. All mice treated with ANG II developed an aneurysm that was consistently localized to the suprarenal region of the abdominal aorta. None of the vehicle-treated mice developed aneurysms. The average outer diameter of the suprarenal aorta from the ANG II-treated group was about twofold greater than those from the vehicle group 1.9 0.2 vs. 0.89 0.02 mm, n 712 group, P 0.001 ; . Compared with vehicle, the ANG II-treated mice showed a 28% increase in heart weight 178 3 vs. 139 3 mg, n 1618 group, P 0.001 ; and a 6% decrease in body weight 27.5 0.3 vs. 29.3 0.6 g, n 1620 group, P 0.01 ; . Thus, the ratio of heart body wt was 35% greater in the ANG II group than in the vehicle group 6.5 0.1 vs. 4.8 0.1, P 0.001 ; . There were no significant changes in the serum levels of total cholesterol, low-density lipoprotein, glucose, or insulin between the ANG II and vehicle groups data not shown. 5. 6. 7.
