The enzyme encoded by this gene mediates the metabolism of 25% to 30% of all therapeutically important medications, 138 including cardiovascular agents and almost all psychotropic drugs.139, 140 Individuals deficient in this enzyme are slow metabolizers, while others may range from intermediate to ultrarapid metabolizers. The CYP2D6 gene has many variations that may result in an enzyme with absent, reduced, or enhanced metabolic activity. These differences affect how rapidly a drug is metabolized, leading to unexpected drug effects, or altered frequency of side effects.75, 123, 140 Differences in the frequency and expression of variations in the CYP2D6 gene have been widely reported within and across racial and ethnic groups.121, 123, 124, 140.

IN SEARCH OF THE IDEAL SCHEDULE FOR THE TREATMENT-NAI VE PATIENT The need for more information regarding the most effective antiviral treatment schedule for the nave patient with chronic hepatitis C has recently been addressed by a study of PEG-IFN alfa-2a PEG 40 kD plus ribavirin, reported by Hadziyannis et al, at the European Association for the Study of the Liver, 37th annual meeting, April 1821, 2002, Madrid Spain.10 In this "optimization" trial, yet to be published, 1284 patients see Table 2 ; from 91 centers across North and South America, Europe, Australia, New Zealand, and Asia were randomized to receive one of four treatments: 1 ; PEG-IFN alfa-2a 180 g plus ribavirin 800 mg day for 24 weeks, 2 ; PEG-IFN alfa-2a 180 g plus ribavirin 800 mg day for 48 weeks, 3 ; PEG-IFN alfa-2a 180 g plus ribavirin 10001200 mg day for 24 weeks, or 4 ; PEG-IFN alfa-2a 180 g plus ribavirin 10001200 mg day for 48 weeks. Randomized patients were stratified by HCV genotype 1 vs. non-1 ; and viral load 800, 000 vs. 800, 000 IU mL ; , with a pre-planned 1: 4: distribution of difficult-to-treat cases genotype 1 and or high viral load ; in the 4 arms. Because of this randomization scheme, overall results are not applicable, and the response can only be assessed by genotype and viral load. Patients infected with HCV genotype 1 Fig. 4A ; achieved the highest rates of SVR 51% ; when treated for 48 weeks with 10001200 mg day of ribavirin. All patients with genotype 1, at any viral load level, showed maximum benefit with the long-term, high-dose treat.

After 15 min on ice, extracts were sonicated 3 10 sec ; , insoluble material was removed by centrifugation 15 min at 12, 000 g ; , and protein concentration in the supernatant was determined using the Bio-Rad Laboratories, Inc. protein assay kit. The protein extracts were mixed with 1 4 vol of 5 electrophoresis sample buffer and boiled for 5 min at 90 C. Protein extract 50 g per lane ; was then fractionated by SDS-PAGE, transferred to polyvinylidene difluoride membrane, and probed with antibodies. Primary antibody was detected by horseradish peroxidase-conjugated second antibody and visualized using enhanced SuperSignal West Pico Chemiluminescent Substrate. The band density of exposed films was evaluated with ImageJ software : rsb .nih.gov ij ; . Immunoprecipitation For immunoprecipitation, 500 g whole cell extract was diluted to protein concentration of 1 g using PBS containing protease inhibitor cocktail and incubated with 5 l antirabbit IgG and 20 l protein G-agarose beads for 1 h at After centrifugation at 12, 000 g for 15 sec, the precleared supernatants were incubated with 5 l anti-ER antibody overnight at 4 C, followed by another 1-h incubation with 30 l protein G-agarose beads. The beads were then pelleted by brief centrifugation, washed three times with PBS and once with PBS containing 0.4 M NaCl, and resuspended in 30 l SDS-PAGE loading buffer for SDS-PAGE and Western blotting. Cell Proliferation Assays To assess the effects of estradiol, ICI 182, 780, or 4-OHT on cell proliferation, cells 1000 well ; were plated in 96-well dishes in hormone-free medium for 3 d before drug exposure. For time-response analysis, cell numbers were determined by MTT assay 62 ; at indicated times after drug treatment; and for dose-response analysis, cell number was determined by MTT assay at d 7 and qvar.

Edwards prosthesis, which is known to have a higher gradient than many other devices.28 3' In our series, no patient received a Starr-Edwards prosthesis. Improvements in surgical techniques in recent years may have contributed significantly to the better longterm results in patients in our study and others6' 7 as compared with those in the study of Henry et al., who underwent surgery between 1972 and 1977. None of the patients analyzed by Henry et al. were operated on under cold potassium cardioplegia, a technique superior to other methods of myocardial preservation, 32 33 whereas that was the case in all of our patients and in many patients in other series we have cited. Finally, there is the broader issue of discrepancy between our results and those of earlier nonechocardiographic investigations relating preoperative left ventricular dysfunction to a less-than-ideal postoperative outcome after aortic valve replacement. Studies have looked at the significance of functional class and heart size, 34 35 angiographic indexes of left ventricular function, 4 36 and other parameters. They have shown, in general, that the more severe the symptoms of the patient, the larger the heart size, and the more compromised the left ventricular function, the worse the result after surgery. These findings have been the rationale and ramelteon.

History of Questran

Active transport of iodide into the thyroid gland is a crucial and rate-limiting step in the biosynthesis of thyroid hormones that play an important role in the metabolism, growth, and maturation of a variety of organ systems, in particular the nervous system 1 ; . Although it has been known for decades that iodide transport into the thyroid gland is mediated by a specific sodium-dependent iodide transporter located at the basolateral membrane of thyroid follicular cells, the sodium iodide symporter NIS ; gene was cloned just 4 yr ago 2, 3 ; . After cloning of the rat sodium iodide symporter rNIS ; from a Fisher rat thyroid line FRTL-5 ; derived complementary DNA cDNA ; library 2 ; , the human sodium iodide symporter hNIS ; was cloned from a human thyroid cDNA library in 1996 3 ; . The hNIS gene is localized on chromosome 19p1213.2 and encodes a glycoprotein of 643 amino acids aa ; with a molecular mass of approximately 70 90 kDa. The coding region of hNIS contains 15 exons interrupted by 14 introns and codes for a 3.9-kb messenger ribonucleic acid mRNA ; transcript 4 ; . As member of the sodium-dependent transporter family, NIS is an intrinsic membrane protein with 13 putative transmembrane domains, an extracellular amino-terminus, and an intracellular carboxyl-terminus. The NIS protein has three potential Nlinked glycosylation sites; 1 is located in the fourth extracellular seventh extramembranous domain ; , and 2 are located in the last extracellular 13th extramembranous domain ; loop 5 ; Fig. 1 ; . NIS cotransports two sodium ions along with one iodide ion, with the transmembrane sodium gradient serving as the driving force for iodide uptake. The sodium gradient providing the energy for this transfer is generated by the ouabain-sensitive Na K -adenosine triphosphatase Na K -ATPase ; . NIS-mediated iodide transport is, therefore, inhibited by the Na K -ATPase inhibitor ouabain as well as by the competitive inhibitors thiocyanate and perchlorate 1 ; Fig. 2 ; . After active transport across the basolateral membrane of thyroid follicular cells, iodide is translocated across and raspberry.
The bottom of the experimental containers and were rendered unavailable as a potential food organism, while contributing to the final prey count. This could explain the data for the trial where the largest medusa produced one of the lowest feeding rates. From these mortality trials, where the Artemia were placed in water that had been occupied by jellyfish for periods ranging from 2 to 24 hours, we estimated that the Chrysaora toxin, or a metabolite, contributed an additional S-20% mortality to the final prey concentration. However, since only a superficial examination of this problem was performed, the rates of consumption reported here have not been adjusted to compensate for prey mortality. Discussion Although the interrelationship between feeding rate, prey concentration and medusa volume is almost linear, less confidence is placed in the values at high prey concentrations, where the toxic effects from the metabolites of large numbers of prey would most influence sampling error. Perhaps a similar study with flowing water incorporated into the experimental design might alleviate this problem. There should be a prey concentration at which the jellyfish is saturated with food and at which point the rate of consumption would cease to increase. This would presumably be a function of the total area of the oral arms and tentacles which varies among medusae. Either we did not present them with this level of saturation or the toxic effect distorted the values at high prey concentrations. However, assuming that this situation never exists in the natural environment, it is unlikely to bear upon estimates of feeding rates and energy flow in the Patuxent River. From the linear regression equation of Fig. 1, a prediction can be made of the predation on Acartia by Chrysaora in the Patuxent River. Heinle et al. 1974 ; report that the average density of Acartia tonsa for the months of July and August in 1973, in the vicinity of Calvert Cliffs, is 65 copepods per liter. Although this is outside the river, the proximity of the location permits a useful comparison. ; Using this and an average medusa volume of 114.4 ml for the Chrysaora population, approximately 18, 800 copepods would be consumed per day by one jellyfish. This figure may be somewhat conservative when we consider that the average volume of the jellyfish prob and questran.

Pregnancy reduces a woman's immunity to malaria and HIV . Thus, avoiding pregnancy through use of family planning will help combat these diseases . Promoting methods of family planning such as the male and female condom which also offer protection from HIV, helps combat STIs directly and indirectly by reducing the stigma of accessing STI services.

By Liz Highleyman Pruritus is itching that may be localized to a specific part of the body, such as the palms of the hands and soles of the feet, or may be a more generalized all-over itchy feeling. Some people even report that it feels like their internal organs itch. Itching is common in people with chronic hepatitis C, especially those with advanced liver disease and cirrhosis. Experts believe pruritus in people with liver disease is due to the accumulation of toxins such as bilirubin ; that are not effectively processed or filtered by the damaged liver. One function of the liver is the production of bile, which helps digest fats. Cholestasis, or blockage of the flow of bile through the liver, can result in a build-up of bile acids and bilirubin in the blood. High bilirubin levels cause jaundice yellowing of the skin and eyes ; , and pruritus is common in people with jaundice. Certain extrahepatic outside the liver ; conditions associated with HCV, such as autoimmune conditions, may also lead to itching. More commonly, itching due to dry skin can be a side effect of treatment with interferon ribavirin; this is not the same as pruritus due to advanced liver damage. Pruritus symptoms can range from annoying mild itching to severe itching that interferes with daily life. Often the itching is worse at night, and may prevent sleep. Simple scratching typically does not relieve pruritus. As a result, some people risk skin infection and injury by scratching themselves with sharp objects. Use of moisturizing lotion, baby oil, or petroleum jelly can help relieve itching due to dry skin. Apply these after a bath or shower to hold in moisture. Some people also find oatmeal baths soothing. Drinking enough water can also help prevent skin dryness. Soft, loose clothing may help, as well as a comfortable climate that is neither too hot nor too cold. Signs of infection redness, pain, swelling, and accumulation of pus ; should be reported to a healthcare provider and, if necessary, be treated with antibiotics. Certain drugs can help reduce itching. Some people find that antihistamines, such as diphenhydramine Benadryl ; or hydroxyzine Atarax ; , help relieve symptoms and allow better sleep. For pruritus due to cholestasis, cholestyramine Questran ; and colestipol Colestid ; may be effective. These drugs are bile acid binders that attach to bile acids in the blood and help eliminate them from the body. They can also interfere with the absorption of other medications, so other drugs should be taken two hours before or after bile acid binders. Some studies have shown that opiate antagonists such as naloxone Narcan ; , naltrexone Revia ; , and nalmefene Revex ; --which are used to block the effects of opiate drugs--can also reduce severe itching. Rifampin, phenobarbital Luminal ; , ondansetron Zofran ; , and ursodiol Actigall ; may also be used, and several other medications are under study. Experimental treatments for pruritus include plasmapheresis in which blood plasma is removed, filtered, and returned to the body ; and ultraviolet UV ; light therapy. Liver transplant is the only cure for severe itching in people with advanced liver disease. For most people with less advanced hepatitis C, though, practical measures and medications are often sufficient to overcome the itch and quinidine.
