AIM: To prospectively present our initial experience with totally laparoscopic transhiatal esophagogastrectomies for benign diseases of the cardia and distal esophagus. METHODS: Laparoscopic gastric mobilization and tubularization combined with transhiatal esophageal dissection and intrathoracic esophagogastric anastomosis accomplished by a circular stapler was done in 3 patients. There were 2 females and 1 male patient with a mean age of 73 5 years. RESULTS: Two patients were operated on due to benign stromal tumor of the cardia and one patient had severe oesophageal peptic stenosis. Mean blood loss was 47 15 mL and mean operating time was 130 10 min. There were no cases that required conversion to laparotomy. All patients were extubated immediately after surgery. Soft diet intake and ambulation times were 5.1 0.4 d and 2.6 0.6 d, respectively. There were no intraoperative and postoperative complications and there were no perioperative deaths. The average length of hospital stay was 9.3 3 d. All procedures were curative and all resected margins were tumor free. The mean number of retrieved lymph nodes was 18 8. CONCLUSION: Laparoscopic transhiatal esophagogastrectomy for benign lesions has good effects and proves feasible and safe and pyrimethamine.

Its use has revolutionized the treatment of nematode and arthropod parasites in animals and has provided hope for the control or even eradication of filariases in humans. Although much remains to be learned about how the drug works and how resistance to it will develop, it has earned the title of `wonder drug'. Origin of ivermectin Ivermectin, the first commercially available macrocyclic lactone endectocide, was discovered in a screening program at Merck : merck ; in the mid-1970s [1]. The origin of the drug perhaps foretold its remarkable impact on both veterinary and human medical practice. Historically, drugs derived from bacterial or fungal fermentations had found application as anticancer or antibacterial agents. The micromorphological changes in the structures of Toxocara canis under the action of 14, 4 mg kg of pyrantel pamoate in vivo were studied on the 8th, 12th, 24th and 36th hours post treatment. Eight naturally infected puppies were treated with pyrantel pamoate and the eliminated parasitic nematodes were collected for histological studies. The results show that the anthelmintics applied cause serious changes in the structure of intestinal cell, hypodermics and muscle cell T. canis. The changes may be described as destructive, degenerative and necrotic processes. The intestines are predominantly affected by the drug. The micromorphological changes in intestinal cells were found on the 8th hour of the action of pyrantel pamoate, and by the 36th hour the intestinal cells had been completely destroyed. The muscle cells showed vacuolization in the cytoplasmic portion on the 12th hour of the action of pyrantel pamoate, where as the fibrilian region of the muscle cells remained unchanged. The cuticle and the endothelial cells of the sexual tubes of both sexes indicated only a weak vacuolization and maintained their structure throughout the experiment. The fine structure of the nerve cords and the sexual products them selves showed no noticeable changes at all. It is concluded that T. canis worms had taken up pyrantel pamoate via an oral route, resulting in initial intestinal changes.
