ROGRESSIVE, irreversible fibrosis is one of the most clinically significant sequelae of ionizing radiation in normal tissues. In the case of the lung, the dose limitations imposed by normal tissue tolerance presently preclude curative radiotherapy for all but the smallest and most responsive tumors. Compounding this problem is the fact that radiation-induced pulmonary fibrosis has proved to be refractive to treatment 1 ; . Therefore, methods of preventing or moderating chronic radiation injury in the lung would have important clinical implications. Collagen antagonists have yielded some of the most promising experimental results in this area. Triiodothyronine 2, 3 ; , colchicine 4 ; , beta-aminopropionitrile 5 ; , and penicillamine 6-9 ; have all been reported to ameliorate various connective tissue reactions in irradiated animal lungs. However, toxicity limits the clinical potential of some of these agents. To our knowledge, only triiodothyronine has been tested in radiotherapy patients 10 ; . D-penicillamine, an inhibitor of collagen maturation, has been used successfully to treat several fibrotic lung diseases in humans 1 1-15 ; , and has been found by us to moderate radiation-induced pulmonary histopathology 6, 7 ; , hypoperfusion 8 ; , and lethality 9 ; in rodents at doses that appear to be free of deleterious side effects. In the present study, we investigated whether penicillamine could modify collagen accumulation in irradiated rat lung and pentamidine.

We describe a 45-year-old woman with minute signet ring cell carcinoma occurring in a gastric hyperplastic polyp. A biopsy specimen obtained from the gastric hyperplastic polyp revealed signet ring cell carcinoma. Endoscopic mucosal resection EMR ; was performed to confirm the diagnosis. Histological examination of the EMR specimen revealed focal signet ring cell carcinoma in the hyperplastic polyp. There are few cases of gastric hyperplastic polyp associated with signet ring cell carcinoma and pentasa.

During the period from May 1, 1999, to December 17, 1999, the MTF and TMCs provided 6, 924 visits for low back pain. Nearly one-half of these visits 47 percent ; were one-time-only visits. In addition, there were about 900 visits to the ER for low back pain. Overall, there were 1.5 clinic visits per low back pain patient during this period. For patients who had more than one visit, the number of visits was 2.7 visits per patient. The average number of visits varied across the clinics and TMCs, ranging from 1.32 to 1.72 visits per patient. In general, TMCs had higher numbers of visits per patient than the MTF clinics. This monitoring also identified a small number of patients six ; with greater than 10 visits. The implementation team plans to follow up on these patients to identify the reasons for such high utilization. For the chart review, the implementation team attempted to pull the medical records for a sample of 391 low back pain patients with a visit between May 1, 1999, and November 2, 1999. DISCUSSION The modulation of Se-GSHPx activity by xenobiotics is of interest because Se-GSHPx degrades cytotoxic lipid hydroperoxides and hydrogen peroxide and, therefore, plays a central role in cellular antioxidant defense. Many xenobiotics have been shown to modulate SeGSHPx activity. Aurothioglucose 29 ; , penicillamine 30 ; , misonidazole 31 ; , mercaptans 32 ; and some peroxisome proliferators [clofibrate, di- 2-ethylhexyl ; phthalate 33 ; , fenofibrate 34 ; and nafenopin 35 ; ] all decrease the activity of Se-GSHPx. In contrast, oxidized lipids 36 ; , high oxygen tension 37 ; and cigarette smoke 38 ; increase Se-GSHPx activity. Here we report that PFDA also increases Se-GSHPx activity at certain di etary Se concentrations. Vortices are one of the most interesting and important features of the ocean and atmosphere. They are also numerous, which alone makes them worth studying. The examples range from Gulf Stream "rings" and Mediterranean "meddies" to tropical typhoons and the Great Red Spot of Jupiter. The aim of this project is to resolve the long-standing contradiction between the observed longevity of oceanic vortices and theoretical studies which all indicate that vortices are unstable. The model under consideration is that of a two-layer ocean with a thin upper layer. We begin by employing the approximate quasigeostrophic equations which are valid for weak vortices only ; on the rotating plane and reduce these equations, coupled with appropriate boundary conditions, to an eigenvalue problem which can then be solved through the use of an appropriate numerical method. Finally, to examine strong oceanic vortices, it is proposed to examine the problem using the exact nonquasigeostrophic ; governing equations and compare the results to those of the quasigeostrophic problem and pennyroyal.
