Lthough almost everyone has experienced hiccups, little is known about their cause or significance. In contrast to reflexes such as coughing and sneezing, hiccups do not appear to have a useful function. Some scientists suggest that the mechanism is a remnant of our evolution from amphibians.1 The term "hiccup" is thought to be derived from the sound of the event. The medical term is "singultus". Episodes range from transient minor attacks to protracted bouts that can continue for days or weeks. The longest recorded bout of hiccups lasted for 60 years. Hiccups can occur at any age. Fetuses hiccup in utero and this has given rise to the hypothesis that the hiccup reflex circuitry might be important in the development of a suckling reflex -- the sequence of movements during suckling is similar to hiccuping, with the glottis closing to prevent milk entering the lungs.

Study Selection To be included in our analysis of efficacy, clinical trials of hypericum extract 1 ; had to be randomized, controlled, and double-blinded; 2 ; had to be limited to patients with a depressive disorder; 3 ; had to test hypericum extract alone and not in combination with other antidepressants; and 4 ; had to present original data that had not previously been published. Articles that met these criteria were further analyzed for methodological quality using a standard checklist that included whether the study 1 ; reported baseline demographics of the study groups, 2 ; described steps taken to ensure blinding, 3 ; used a standardized outcome measure as defined by the Consensus Conference on the Methodology of Clinical Trials of Antidepressants, 9 4 ; performed an intention-to-treat analysis; and 5 ; provided a full accounting of all study subject dropouts and reasons for dropping out. This search strategy yielded a total of 388 citations. Twelve of these were clinical trials.10-21 Four of the trials did not meet our inclusion criteria: 2 were not placebo controlled, 18, 19 1 tested hypericum extract in combination with another agent, 20 and 1 presented data that had previously been published.21 The results of the remaining 8 randomized controlled trials are presented below. PHARMACOLOGY Hypericum extract contains at least 10 substances that have been shown to have biological activity, including hypericin, pseudohypericin, xanthones, monoterpenes, -sitosterol, quercetin, and catechin. Many of these substances have been shown to bind neuroreceptors in the brain and to inhibit the uptake of various and feverfew.

Twelve healthy males 33"2 years; range 2350 years ; were studies. They were all normotensive 126"3u 81"2 mmHg ; and with a normal weight BMI 23.6"0.5 kgum2; range 20.826.0 kgum2 ; . None of the subjects were on any medication. The study was approved by the Ethics Committee of the Karolinska Institutet, Stockholm, Sweden. At about 08.00 h, infusions of 20% para-amino hippuric acid PAH, Merck Sharp & Dohme, West Point, USA; 0.2 mlukg body weight, rate 0.5 mlumin ; and 25% inulin Inutest1, Kemior, Stockholm, Sweden; 0.2 mlukg body weight, rate 0.5 mlumin ; were started.

DISCUSSION Literature survey revealed that fenugreek seeds possess a plethora of benefits under various experimental conditions. The seeds possess significant antidiabetic Sharma et al., 1996 ; , antiatherosclerotic Sharma et al., 1996 ; , antiinflammatory Thakur et al., 1994 ; , antinociceptive Puri, 1998 ; , antiulcerogenic Suja Pandian et al., 2002 ; and antineoplastic effects Sur et al., 2001 ; . However, there is limited information concerning its protection against EtOH toxicity. In a previous study, we reported that fenugreek seed administration reduced the increase in the levels of biomarkers of oxidative stress in the liver of rats dosed with EtOH Thirunavukkarasu et al., 2003 ; . Other than this, there are no reports concerning the effects of fenugreek seeds on the cytotoxicity induced by EtOH. This study provides evidence that fenugreek seeds offer a strong protective effect against EtOH-induced cytotoxicity in Chang liver cells. We used Chang liver cells as a non-malignant cell model for studying EtOH toxicity. Cameron et al. 1998 ; reported that hepatoblastoma cells HepG2 ; exposed to 6080 mM EtOH for 24 h show cytotoxicity, which was dose-dependent. Walker et al. 1974 ; obtained cytotoxicity in normal human liver cells after exposure to 69174 mM EtOH. In our study, the cytotoxic effect in Chang liver cells was obtained after 24 h exposure to 30 mM EtOH. The cytotoxic dose of EtOH to hepatocytes differs between the cell strains and can probably be accounted for by a variety of mechanisms, which are due to the differences in the metabolism of EtOH by alcohol dehydrogenase and induction of cytochrome P450 Wu and Cederbaum, 1996 ; . EtOH markedly inhibited the viability of Chang liver cells and this was a consequence of the induction of apoptosis as ascertained by flow cytometry. EtOH treatment caused and flax.

Inducers are medications that result in increased blood flow to the liver or initiate synthesis of additional P-450 enzymes. These actions result in increased metabolic activity of the isoenzymes. Medications that are considered P-450 inhibitors do so by either competing with other substrates for binding at the enzyme or inactivating the enzyme.2 Not only do medications influence metabolic activity, but genetic disposition can also affect the activity of certain P-450 enzymes. CYP 1A2 metabolism is decreased in 13% of Caucasians, African Americans, and Asians. Approximately 10% of Caucasians are considered to be poor 2D6 metabolizers, while only 2% of African Americans and Asians have this genetic feature. Five percent of Caucasians and African Americans demonstrate poor metabolism through the 2C19 pathway, but as many as 23% of Asians are poor metabolizers of this enzyme.1, 9 Multiple P-450 pathways metabolize many antidepressants, and 2D6 and 3A4 are the most common of the pathways. A number of antidepressants will inhibit these pathways as well Table 2 ; . When reviewing this information for the first time, clinicians may wonder how to determine which potential drug interactions are more and flecainide.
