79 Seizure data on the ecstasy group of substances substantiate the trend already noted in the chapter on illicit manufacture above: the enormous increase of illicit manufacture in Europe. The largest growth rates within the ecstasy group are reported for MDMA. According to Interpol data, MDMA already accounted for 95% of all ecstasy seizures in units over the 1992-1994 period. For seizures reported in kilograms, the relative importance is less, though rapidly growing. Figure 66 shows the increase in major seizures of MDMA in Europe from 1991 to 1994. Growth rates clearly exceed those of amphetamine, cocaine or heroin. Figure 67, based on Interpol data, shows that major seizures of the ecstasy group MDMA, MDA, MDEA ; in Europe are already larger than those of amphetamine averaged over 1992-1994.

Assessment of intestinal permeability by quantifying unmediated absorption of at least two sugars of different sizes provides a sensitive index of intestinal damage [9]. We used lactulose as the disaccharide probe, and rhamnose and mannitol as two alternative monosaccharide probes in a hypo-osmolar formulation. Both rhamnose and mannitol have been widely used and provide similar information regarding changes in ` paracellular pathways '. However, as there are minor differences in their processing in vivo [9], both sugars were included in the test drink. Following an overnight fast, subjects emptied their bladders and then drank a standardized sugar solution containing 5 g of lactulose, 2 g of mannitol and 1 g of rhamnose in a total of 450 ml of water calculated osmolality 69 osmol\kg of water ; . Subjects were allowed unlimited intake of fluid after the first hour of the test to ensure adequate urine output. The urine was collected and pooled over the next 5 h and total volume recorded. Aliquots were centrifuged briefly to remove gross debris and the supernatant frozen at k25 mC until later analysis. Analyses of sugar content within the urine were based on the method of Srensen et al. [10], with minor modifications. The various sugars were separated using HPLC and quantified using a pulsed amphometric detector. Using this technique, sugars are oxidized on the gold electrode at the working potential Eh l 0.05 V ; , ! the current produced being a measure of the amount of sugar present in the sample [11]. The system comprised of a Hewlett Packard 1100 series HPLC system using a Dionex CarboPac PA10 anion-exchange analytical column 4.5i250 mm ; with an equivalent guard column. NaOH 50 mM, low in carbonate ; BDHMerck Ltd ; was used as the mobile phase 1 ml\min, isocratic conditions ; . Sugars were detected using a Hewlett Packard 1049A Electrochemical Detector with a gold working electrode and solid reference electrode. The potentials were set as follows : Eh l 0.05 V, Eh l 0.6 V and Eh lk0.8 V ; and t l ! 120 ms, t l 120 ms, t l 400 ms. Data analyses were # $ performed using the Hewlett Packard Chemstation software. Typically, the mannitol peak eluted from the column at approx. 2 min, rhamnose at 4 min and lactulose at 9 min. Analysis of single- and mixed-sugar standards in the concentration range 0.05 mg\ml to 50 mg\ml showed.

Appreciate that the cause of acute eosinophilic pneumonia is often unknown. 2. Identify methods of diagnosing acute eosinophilic pneumonia. 3. Recognize that intravenous or oral steroid therapy is the only confirmed treatment for acute eosinophilic pneumonia. CASE: A 24 year old woman without any significant past medical history initially presented to an emergency room with persistent symptoms of dyspnea, non-productive cough, low-grade temperatures, and general malaise. She was discharged twice with azithromycin for a presumed atypical pneumonia. On her third visit, she was found to be in respiratory failure with radiographic evidence of diffuse bilateral airway space disease and was intubated immediately. Upon arrival to the intensive care unit, physical examination was remarkable for bilateral rhonchi, diminished lung sounds, and thick respiratory secretions. Basic laboratory data was within normal limits. Further radiographic studies of the chest demonstrated extensive bilateral fluffy infiltrates in the lower lobes and small bilateral pleural effusions. She was initially treated with broad spectrum antibiotics and antifungals for a presumed respiratory infection and continued on ventilator support. Despite the additional support, the patient deteriorated clinically and needed additional FIO2 and PEEP. Eventually, she underwent fiberoptic bronchoscopy and a bronchoalveolar lavage was performed. Although the culture data was negative, the cell count differential on the BAL was greater than 64% eosinophils which was consistent with acute eosinophilic pneumonia. She was placed on intravenous high dose steroids and had significant improvement in her pulmonary mechanics allowing her to be extubated. Her antimicrobial therapy was stopped and she was eventually weaned completely from oxygenation as her chest x-ray and physical examination were both consistent with a cleared respiratory process. She was discharged in stable condition with a prednisone taper over a one month period. DISCUSSION: This case demonstrates acute eosinophilic pneumonia which was first described as a cause of respiratory failure in 1989 and is usually a diagnosis of exclusion. Possible contributing causes include unusual outdoor activities, cigarette smoke, exposure to fine airborne sand or dust, or possibly some other occult environmental factor. Clinical features of acute eosinophilic pneumonia include an acute febrile illness of less than three weeks duration, nonproductive cough, dyspnea, myalgias, night sweats, and pleuritic chest pain. Key physical examination findings include fever, tachypnea, and bibasilar inspiratory crackles or rhonchi. Respiratory failure is a common finding upon presentation and often requires mechanical ventilation. Routine laboratory studies are nonspecific and generally not helpful. Radiographic findings include bilateral diffuse mixed alveolar and reticular opacities with small bilateral pleural effusions. Analysis of bronchoalveolar lavage fluid often shows a very high percentage of eosinophils. Treatment uniformly consists of intravenous or corticosteroid therapy. The response is often dramatic, occurring within 12 to 48 hours, and there is no relapse following withdrawal of the steroids. In the absence of respiratory failure, initial treatment is oral prednisone. In the presence of respiratory failure, methyprednisone is given until respiratory failure resolves, after which oral prednisone may be used and dok.
