PSA is increasing feedback mechanisms so that advocacy efforts are responsive to the needs of physicians and the Federation. The new information posted on the site, such as the Health Plan Complaint Form, will allow PSA to gather useful information from the Federation and individual physicians in order to track trends within the health care marketplace. Through the revised web site, PSA provides tools for physicians to deal with the marketplace, keeps physicians upto-date on market trends, and shares many of the AMA's ongoing private sector initiatives. There are several new postings on the site, including a story educating physicians about how one physician took on "Big Insurance" and won. The site also includes helpful hints and tools for physicians in the event they would like to pursue legal action against an insurer. PSA will continue to improve and update the site with the latest information on the most pressing health care issues facing physicians in the private sector today. CAPITATION GUIDE The issues of physician bankruptcies and the inherent problems with the contractually agreed-upon capitated rates in California were brought forth by the California Medical Association CMA ; , which asked for the AMA's assistance in determining the factors that would go into an "actuarially sound" capitation rate. Specifically, CMA asked the AMA to assist in an effort to study and then compare California capitation rates and practices with other states to assist physicians in their evaluation of the financial viability of capitated contractual rates offered by health plans. The AMA, through Private Sector Advocacy and the Center for Health Policy Research, organized a joint research and analysis project with CMA to explore the possibility of providing physicians with the resources and tools to evaluate proposed, as well as existing, capitated rates within contractual arrangements. The culmination of this joint project is the development of a guidebook, preliminarily entitled "The Physicians' Capitation Guidebook: Benchmarking Capitation Rates, " that physicians can use to actuarially determine financially viable capitation rates for contractual arrangements. This Guidebook will allow the physician or physician groups the option of calculating a benchmark per-member-per-month PMPM ; rate for a non-elderly population based either on average fee-for-service payments made for a commercial population or on 100% of the 1999 Medicare Resource Based Relative Value Scale. This Guidebook details the variations in capitated contractual arrangements and provides the flexibility necessary to calculate a benchmark PMPM for virtually every form of capitation, from a straight forward primary care services agreement to a more complex global contractual arrangement that includes all physician services, administrative activities, pharmacy risk, and other delegated functions. EMPLOYER OUTREACH INITIATIVE AMA has adopted a plan to expand health insurance coverage and promote increased patient choice of plans and providers. PSA has implemented a campaign to educate employers and encourage them to promote various aspects of this plan. As the largest purchaser of health insurance in this country, the employer community is uniquely positioned to play a major role in any true health insurance reform. Considerable networking with large and small employers representing large segments of the employed population, has taken place and target lists for future contacts have been developed. It is anticipated that employer outreach activities will be more fruitful once the national Patients' Bill of Rights legislation is resolved. Employers are leading the way in many quality initiatives, including developing indicators of plan physicians' quality performance. PSA is currently monitoring the development of these physician "quality" indicators, and is also compiling informational resources about the use of report cards by employers and health plans. While quality is a top priority for the AMA, some of these initiatives are often linked to profiling of "outliers" and used for deselecting physicians from plan networks. It is imperative to keep the focus on quality issues, not on punishing physicians by deselecting them from health plan networks. EDUCATION OF RESIDENTS & EMPLOYED PHYSICIANS REGARDING THEIR ORGANIZING OPTIONS PSA continues to educate residents and employed physicians about their organizing options. In addition, PSA continues to promote Physicians for Responsible Negotiation PRN ; to the Federation and provides appropriate resources to PRN staff as they develop new marketing strategies to attract sustaining members and cyclosporine.
