Background When the Medicare program was first implemented, it paid for hospital services inpatient and outpatient ; based on hospital-specific reasonable costs attributable to furnishing services to Medicare beneficiaries. Later the law was amended to limit payment to the lesser of the hospital's reasonable cost or customary charges for services furnished to Medicare beneficiaries. Specific service-based methodologies were then developed for certain types of services, such as clinical laboratory tests and durable medical equipment, while payments for outpatient surgical procedures and other diagnostic tests were based on a blend of the hospital's aggregate Medicare costs for these services and Medicare's payment for similar services in other ambulatory settings. While this mix of different payment methodologies was in use, hospital outpatient services were growing rapidly following the implementation of the IPPS in 1983. The brisk increase in hospital outpatient services led to an interest in creating payment incentives to promote more efficient delivery of hospital outpatient services through a Medicare prospective payment system for hospital outpatient services, and the final statutory requirements for the OPPS were established by the BBA and the BBRA. During the period of time when different approaches to prospective payment for hospital outpatient services were being considered, a variety of reports to Congress June 1988, September 1990, and March 1995 ; discussed three major issues related to defining the unit of payment for the. Preheat a nonstick pan over high heat for scallops. 4. Make the crab dip: Break up crabmeat with your fingertips and check for pieces of shell. Using a rubber spatula or wooden spoon, combine crab with Old Bay, cream cheese, sour cream, chives, lemon juice, and Tabasco. Season dip with salt and pepper, to taste. Transfer to a small bowl to serve. 5. Season all the scallops with salt and pepper. To very hot skillet, add 1 tablespoon EVOO once around the pan ; , then place 9 scallops in the pan, and inch apart. Sear scallops 2 minutes on each side, until caramelized and opaque.
