To mention the limitations of this study, firstly, some factors were not assessed. According to the behavioral model by Anderson [17], factors affecting eye care service utilization can be divided into three categories: predisposing, enabling, and need. In this study, some predisposing factors demographic variables of age, gender, education, marital status, and religion ; and the factor of need were assessed, but not the socioeconomic status or non-behavioral factors such as access to services, their cost, and the attitude of service providers. The extent of the problem in the studied population urges the need for exhaustive studies to identify influential factors more accurately. Secondly, despite a reasonable response rate of 70.3%, people's access to eye care services may be different between Tehran and the study population. However, it can be assumed that people with eye conditions and those who sought eye care before had a relatively higher participation rate, and so the situation may be even worse in reality. Thirdly, we were constrained by the sample size and the number of cases for each type of eye disorder to expand our analysis to details and we did not evaluate the utilization of each type of ophthalmic disorder individually. It is worth noting that there is no data about the proportion of people that have been previously diagnosed, but have not utilized care despite the diagnosis. Fourthly, from a socio-economic point of view, the study sample, consisting of Tehran inhabitants, does not represent the country population, and study results cannot be generalized. It is therefore necessary to carry out similar studies in other parts of the country. Fifthly, we did not specify whether the eye care provider for each visit was an ophthalmologist or an optometrist. In addition, the study is based on participants' reports which could have been influenced by their judgment on the specialty of the eye care provider or their ability to recall it. Such a limitation does not apply to studies in which data was collected from past records or documenting the providers [6, 13].

Targeted audiences are assumed to be advanced high school students or those with higher education. More likely, enzyme monomers bind to the phosphate backbone of each DNA strand thus explaining the binding stoichiometry and the ability of the enzyme to bind ssDNA with 1: stoichiometry. The fact that the E9 DNase prefers to bind and hydrolyse ; DNA above a certain length has two important consequences for our understanding of colicin action. First, it implies that the free energy of binding is derived largely from phosphate recognition and that this may play a role in the hydrolysis of DNA, observations consistent with the natural substrate for colicin DNases being the bacterial chromosome. Second, this has ramications for the way in which DNase colicins are inhibited in bacteria. Suicide is prevented in colicin-producing cells through the action of an immunity protein which binds with very high afnity the Kd for Im9 binding the E9 DNase is 1016 M at pH and 25 C33 ; to an Immunity Protein Exosite IPE ; that lies adjacent to the H-N-H motif, with the immunity protein making no direct interactions with catalytic residues.10, 34, 35 The immunity protein is thought to inhibit the enzyme primarily through steric hindrance and electrostatic repulsion of substrate binding and this is supported by modelling studies, 10 and by the absence of conformational changes in the active site on immunity binding as reported by NMR spectroscopy.36 Hence, it would be a disadvantage to a colicin-producing organism for the enzyme to retain endonuclease activity against small, single-stranded oligonucleotide substrates since the immunity protein would not inhibit their binding. Interventions. Endometrial ablation using NovaSure System. Methods. The NovaSure System was used to treat 107 patients with menorrhagia secondary to DUB. PBAC diary sampling was used to qualify the patients for the study, as well as to conduct the post-treatment evaluation of menstrual blood loss and bleeding pattern. No drug or mechanical endometrial pre-treatment was used in this group of patients. Position of the uterus was not a factor in patient selection. All patients received the treatment under intravenous sedation. Results. No intra-operative or post-operative complications were observed. Treatment time averaged 94 seconds. At 12months follow-up 58% of patients reported amenorrhea and 96.1% had reported reduction in bleeding to normal levels or less. Conclusion. Endometrial pretreatment is not required when using the NovaSure system. Active bleeding during the time of the operative visit is not a limiting factor when NovaSure system is used. Long-term follow-up results demonstrate that the NovaSure System is safe and effective and should be considered as treatment of choice in treatment of patients suffering from menorrhagia secondary to DUB.
