Liver fibrosis and cirrhosis are predisposing factors for the development of hepatocellular carcinoma HCC ; . Hemosiderosis has also been described to trigger carcinogenesis. A significant iron overload, as found in hereditary hemochromatosis HHC ; , is a risk factor for HCC and may also promote the symptoms of porphyria cutanea tarda PCT ; . A 68-year old male patient presented to our clinic with a suspected HCC, elevated alpha-fetoprotein but normal liver function tests. He reported a 25 year-old history of vitiligo upon exposure to sunlight. The patient underwent an extended left hemihepatectomy, and the recovery was uneventful, with the exception of a persistent hyperbilirubinemia. Perfusion problems and extrahepatic cholestasis were ruled out by CT-scan with angiography and MR-cholangiopancreatography. However, MRI showed an iron overload. Histology confirmed the HCC pT3, pN0, G3, R0 ; and revealed a portal fibrosis and hemosiderosis. Based on the skin lesions we suspected a PCT that was confirmed by laboratory tests showing elevated porphyrin, uroporphyrin, coproporphyrin and porphobilinogen. Concurrently, molecular diagnostics revealed homozygosity for the C282Y mutation within the hemochromatosis HFE gene. After phlebotomy and normalization of liver function tests the patient was discharged. This is the first case ever showing the unusual combination of HCC in a fibrotic liver with HHC and PCT. This diagnosis not only warrants oncological follow-up but also symptomatic therapy to normalize iron metabolism and thereby improve liver function and alleviate the symptoms of HHC and PCT. Thus progression of fibrosis may be prevented and liver regeneration supported.

I often introduce plants by stating that Native Americans had no word in their vocabulary for "weeds, " as all plants had uses. Even poisonous species were often utilized medicinally, or for hunting and warfare. I challenge my students to choose any plant they consider a weed and to find some human use for it or learn its ecological significance, especially in its place of origin. The further they look, the longer their lists grow. They often discover that different Native tribes had different uses for the same plant. Further, I mention that great mystery awaits explorers of the plant world, as new plants continue to be discovered. Perhaps only ten percent have been listed in botanical literature, and of those less than one percent have been researched for medicinal application. Finally, I emphasize that many "weeds" that we attempt to remove from our lawns and gardens have greater nutrient value than our cultivated plants.

There was a considerable delay for Agenerase to become available, as the last PI to be approved was in 1997. By this time people were in need of a new protease inhibitor since mutations from older PIs were showing up. But only one Agenerase mutation is not cross resistant to other PIs and clinical trial data shows only a modest benefit for those who are salvage patients. So, Agenerase would fall into the "me too" class of drugs offering not much evidence that it is useful in initial or third-line regimens. Shortterm studies have shown no evidence for facial and limb wasting or signs of diabetes, yet showed a 90% increase in triglyceride levels. Now if Glaxo would develop a NNRTI they will corner the market. Then we'll wait for a HAART combination pill. --Matt Sharp!
